Permission form for afternoon excursions

IF YOUR CHILD IS NOT ATTENDING EITHER EXCURSION, THEY MUST BE COLLECTED FROM THE CHURCH BY 12PM THAT DAY.

There will be two excursions as part of the Holiday Bible Club. Monday to Wollongbar Skate Park and Tuesday to a local park for a picnic lunch. You can give permission for niether, one or both here. They will both run from 12 to 3pm.
